If you have lost your divorce decree, the following steps are required to obtain a legal duplicate:
- Online Validation: Verify your decree number via the A.C.O (Online Divorce Certificate & Validation) portal.
- Local Introduction: Obtain a Cover Letter from the RT/RW regarding the lost document.
- Village Verification: Process a loss statement at the Village Office.
- Police Report: Obtain a formal loss report from the local Police station.
- Village Head’s Certificate: Secure a Statement from the Village Head/Lurah intended for the Religious Court (Pengadilan Agama) to request a duplicate. This document must be signed by the applicant, the Village Head, and acknowledged by the local KUA.
- Formal Affidavit: Sign a statement (on a Rp. 10,000 duty stamp) declaring that the lost decree has not been used for remarrying as of the date of the request.
