Marriage Registration and Documentation Forms

Based on the Decree of the Director General of Islamic Community Guidance Number 473 of 2020 regarding the Technical Guidelines for Marriage Registration, there are specific attachments for forms ranging from Model N1 to Model N6.

This regulation introduced several significant changes compared to the previous decree (Number 713 of 2018), particularly regarding the numbering and function of each form. These updates aim to streamline the administrative process at the Village or Sub-district level.

Key Changes in Form Models

Under the 2020 technical guidelines, the following adjustments were made to the form structure:

1. Model N3: Formerly the “Bride/Groom’s Consent Letter,” it is now used as the Application for Marriage Legalization (Isbat). The Consent Letter itself has moved to Model N4.
2. Model N4: Formerly the “Parental Permission Letter,” it is now the Prospective Bride/Groom’s Consent. The Parental Permission Letter has moved to Model N5.
3. New Additions: The regulation also introduced specific formats for Marriage Examination Forms and Guardian Examination Forms.

The standard marriage registration package (NA) used at the Village or Sub-district office now consists of the following models:

Please note that forms N7 (Rejection Form), N8 (Examination Form), N9 (Announcement Form), and N10 (Recommendation Form) are no longer bundled within the basic village registration package as per the latest guidelines.
