Bridging the Healthcare Gap: Driving Universal Health Access Inclusion through Social Safety Net Financing

Equitable healthcare is a fundamental right and a primary obligation of the state. Situbondo Regent Regulation (Perbup) Number 20 of 2021 establishes the guidelines for Situbondo Sehat Gratis (SEHATI), a definitive instrument for Universal Health Access Inclusion. Recognizing that many vulnerable residents remain outside the National Health Insurance (JKN) system, the Situbondo Regency Government has implemented this Social Safety Net Financing mechanism. This ensures that every citizen, especially the impoverished, receives optimal, integrated, and well-targeted medical services without financial barriers.

Program SEHATI acts as a crucial regional buffer, covering health costs for residents who are not currently protected by central or provincial government insurance schemes.

Strategic Objectives of the SEHATI Program

To achieve Universal Health Access Inclusion, the SEHATI program focuses on three core pillars of implementation:

  • Guaranteed Financial Coverage: Ensuring that the cost of medical care is fully covered for the underprivileged residents of Situbondo Regency.
  • Standardized Quality Control: Delivering healthcare services that meet national standards while maintaining strict cost and quality control (kendali mutu dan kendali biaya).
  • Fiscal Accountability: Managing the regional health budget with a high degree of transparency to ensure every rupiah reaches those in need.

Defining the Target Beneficiaries

The success of Social Safety Net Financing depends on precise targeting. The SEHATI program is specifically designed for:

  1. Vulnerable Residents: Impoverished citizens with a Situbondo e-KTP, regardless of whether they are registered in the Integrated Social Welfare Data (DTKS), provided they lack other insurance or are awaiting JKN integration.
  2. Inactive PBI Holders: Residents whose “PBI” insurance cards are inactive and are not in the DTKS but live in poverty.
  3. Maternal and Neonatal Care: Infants needing referral after 28 days and postpartum mothers beyond 42 days from poor families without JKN coverage.
  4. Social Institutions: Students in Islamic boarding schools (Santri) and children in orphanages who are residents of Situbondo Regency.

Conclusion: Strengthening the Regional Welfare System

In conclusion, the Situbondo SEHATI Program is the definitive instrument for Universal Health Access Inclusion and Social Safety Net Financing. It ensures that poverty is never an obstacle to receiving life-saving medical treatment. By integrating this regional program with national insurance goals, the Situbondo Government protects its most vulnerable citizens and builds a more resilient society. A healthy population is the strongest foundation for regional development and social justice.
